Form5 prosthetics is leading the charge in inspiring high-school students to explore the realms of engineering, design, and healthcare.

In 2022, Form5 launched PRE-FAB, a team-based workshop that empowers students to design and construct prosthetic devices for mock-recipients, providing a unique intersection of health and science education.

In 2019, Form5 initiated its CO-FAB®, which stands for collaboration and fabrication, a one-of-a-kind workshop that directly includes individuals with limb differences in product development.

Over a month, a recipient co-creates a conceptual device with teams of students and industry mentors. It is an experiential and life-changing opportunity for each cohort.

Form5 celebrated its 7th CO-FAB Workshop in March 2024, empowering Tatum with bespoke devices to pursue his dream of becoming a hockey player.
