IMPLANTED DEFIBRILLATOR SHOCK OF CONSCIOUS PATIENT. Patient is 19-year-old female with severe hypertrophic cardiomyopathy (HCM). ICD implantation occurred at age 16. Previous set of two shocks administered when patient went into sudden cardiac arrest in her sleep at age 17. Event shown in real time and slow motion.
This event occurred while patient was performing in a lip-sync battle where she had attended high school. Patient reports experiencing disorientation and blurred vision consistent with episodes of elevated heart rate and syncope. However, patient did not realize she was being shocked. Instead patient thought her friend had forcibly hit into her, twice. Patient confronts friend who denies doing so. Since this was a public event, there were videos which showed friend had not made contact. Once seeing, patient realizes what must have occurred and contacts electrophysiologist who confirms. Patient describes sensation as “somebody hitting a homerun on your chest with a sledgehammer”.
Patient attends appointment with electrophysiologist the following day. AICD interrogation shows patients was shocked for what appears to be sinus rhythm to a
rate >188 bpm. (The VF detect had been turned down to 188 bpm due to ventricular tachycardia detected at a rate of 197 bpm). The dectect rate was increased to 200 bpm. Stable lead measurements. Patient was taking and tolerating metoprolol XL 75 mg daily. Dosage increased to 100 mg daily.
ICD in HCM - Who Gets It?
ICD is indicated in HCM patients at high risk for SCD, including those with:
History of SCD or sustained VT/VF
Massive LV hypertrophy (>30mm)
Unexplained syncope
Abnormal blood pressure response to exercise
Family history of SCD
An ICD shock occurs in HCM if VT/VF is detected above a programmed threshold.
The ICD may attempt ATP first before shocking.
ICD therapy is lifesaving in high-risk HCM patients for SCD prevention.

Because the aortic valve opens very well and reaches maximal excursion 🚪⬆️

HeartCharged is supporting an independent, non-interventional research study being conducted by Global Perspectives, an IQVIA business, to better understand the experiences of women living with cardiovascular disease.
The study involves a 90-minute online focus group discussion with other women in the United States who also have cardiovascular disease. Participants will receive an honorarium of $150 USD for completing the focus group and an additional $25 USD for providing confirmation documentation (for example: a physician note, copy of a health record or patient portal screenshot, lab results, or a prescription or medication photo).
Researchers are currently looking to speak with women in the United States who meet at least one of the following criteria (confirmation documentation required):
• Women diagnosed with cardiomyopathy within the last 5 years
• Women diagnosed with high LDL-C (“bad” cholesterol) or elevated lipoprotein(a) (Lp(a) within the last 5 years, with a history of ASCVD and a cardiac event in the last 5 years (for example: heart attack / myocardial infarction, unstable angina, or ischemic stroke)
Please note that no medical treatment is provided as part of this study. Participation is voluntary, and all research is anonymous. No questions will be asked that could identify you personally, and all data will be treated confidentially and analysed in aggregate form.
If you express interest in participating, eligibility will be validated either via a brief phone call (approximately 10 minutes) and/or a short online eligibility survey. This validation step is not remunerated, but is required to confirm eligibility before participation.

🫀 Every time I run a cardiac arrest, there’s a short list in my head—and hypertrophic cardiomyopathy is always on it.
This new data from JAMA Network Open adds important nuance. In ~6000 patients with newly diagnosed HCM, the 10-year risk of out-of-hospital cardiac arrest was higher than controls (~3–4% vs ~1.5–3%)… but still in the low single digits.
👉🏾To me, this feels less like a ticking time bomb
…and more like a chronic, smoldering risk.
🚨 What actually stands out? Heart failure.
• Not subtle
• Not borderline
• ~3x higher risk of OHCA
When these patients decompensate, their risk profile changes—and that matches what we see clinically.
Also worth noting:
Risk increases with age at diagnosis, with older patients carrying the highest burden.
📊 Why this study is strong
• Nationwide cohort (6.4 million people)
• 20+ years of follow-up
• Near-complete OHCA capture
• Incident HCM + matched controls
• Multiple sensitivity analyses
👉🏾 This is real-world, population-level data at scale
⚠️ But important limitations
• HCM defined by ICD codes (no echo, MRI, genotype)
• No phenotype-level risk stratification
• Only resuscitated arrests included → likely underestimates true SCD
• Cause of arrest not confirmed
• Danish system → may not generalize everywhere

Most HCM patients will not arrest
